Does anyone know the name of this plant?

    This plant was given to me last year, it is a perennial. I am in PA in zone 6. It is about 12" tall and has little purple flowers. It just started blooming about two weeks ago. We have it in partial shade. I would really like to know what it is so I can put a name tag on it.     Looks like Jacobs Ladder. Polemonium. Theres also a white flowering form. It readily self sows.
