i'd vote for getting in shape first, as you get in shape, your size will change. you may not lose the 20 lbs, but you should lose 20lbs of fat and gain some muscle. if you just lose the weight, you'll lose muscle, too and that will slow your metabolism and make it harder to lose weight, etc.
start walking and doing general exercises-pushups, crunches, etc. check out the early morning fitness shows on tv, even basic tv usually has one or two, early in the am, or check out some videos from the library or your video store. walking hills/stairs will tone your rear while burning fat. check out some of the many fitness magazines for women for other exercises (prevention, shape, etc) fitness is more important than a few extra pounds,.
