Buck service memo needed!

I have a bred doe probably sold and going home Thursday, and would like to send the buyer with a paper service memo in hand along with all the other documents - ADGA is down so I can't do an online service memo nor print a paper one.

Does anybody have the file saved or a service memo booklet thing? Can you send me a digital copy or take a picture of one so I can just make my own with all the pertinent information? I rarely use them so I don't want to forget anything important. :P

Thanks, that's perfect! I used to have them all saved on my old computer but after the hard drive crashed, I haven't gotten around to doing it on my new one. :P
