You know, milk cows are expensive animals. And they are very important to people. I would call a vet. Loosing a cow isn't like loosing a rabbit, chicken or even a goat. It's worth the cost for a vet.

I had typed a nice response to this, and it vanished. ??

When I had Beefmaster cattle, they would occasionally, step across a stick and get it lodged between their toes. PAINFUL! You've checked, but a stick could have come an gone. If she's not recovered in a couple of days, and you're sure she doesn't have hoof rot, then it's vet time.

Yes, they can twist an ankle. Lots of ligaments in there.

Thanks for your response, I am quite aware. I have several milk cows and many goats. We don't have a "friendly local vet" any longer (he moved) and I have been known to load my cow and take her to the vet. Whether or not to call a vet was not the question I was asking. Thanks again.

I've had a few sprained ankles on cows (these Ozark hills are rocky and steep!). I give them aspirin and watch it. I sometimes wrap an ice pack on while I'm milking. Those tactics probably only help me, but the girls don't seem to mind. I don't think I've ever taken a limping cow to the vet.
