Cow Share Programs

I am just looking for info......... for people that run a cow share program, get milk from one or know someone that has one.......

Does the farmer supply the jars the milk comes in?
Does the farmer make the share holders sign a contract?
What is the average "buy in" fee? Is it returnable?

Any tips or help that anyone wants to share would be great!
