One of the real nasties I've seen happen a couple of times is people going over sideways from something a bit elevated (one was a pair of platform shoes, another was stairs they were standing sideways on). I've heard of others too. What happened was that the tendons stretched so much that they put tension on where they attached to the bone, and actually popped off the flake of bone the tendon was attached to. You had the tendon wobbling around loose inside the foot with a flake of bone on the end of it. Those take nearly forever to heal, particularly if they aren't bandaged right to start with, and the tendon starts to shrink so the bone draws away from where it's supposed to re-attach.
That can be worse than an outright break or even a sprain. If it goes on hurting real bad it may be worth gritting your teeth, spending the money, and going to a doctor.
