it sounded like the egg hatched inside the hen's body and that's what killed the hen

Wouldn't the temperature be just a bit too warm to properly hatch an egg? How about the incorrect humidity, basically that egg was hatched under water.

Not really, fireweed, birds do not develop an external amniotic sac - it's inside the egg not outside, no matter if the egg is hatched in an incubator, under a hen, or inside a hen.
