Root cellar under house with poured cement foundation?

Question - what's the most efficient way to have a root cellar underneath a house with a poured cement foundation? Should it be dug first and then the trapdoor not poured, or poured first with a space to then dig out later?

Or, I guess, I could dig it NEXT to the house, but as long as it is accessible from the inside that's all I care about.

Most efficient thing for you to do might be just make a small insulated room in the northwest corner of the poured concrete foundation. Add a inlet pipe and outlet pipe for
ventilation, thats what worked well for us.
There are some good plans on mother earth news google search it.
Not much extra cost and easy to get to.

A lot of fruits/veggies like a high humidity, which a concrete slab won't supply. Do consider dirt. Otherwise you may end up hiking down water to wet the floor.
